Rotation # 5 – Site Visit Reflection

My site visits for Emergency Medicine were done with Shterni Seligson. During my evalutions, I presented several H&Ps, including one written about a patient that was intoxicated and come to the ED after an assault by multiple people, making the history hard to elicit. My first evaluation contained some simple errors and had more of a focused ROS, which I edited to contain more adequate information. I also did not have as much as a fleshed-out assessment and plan, which I made sure to account for during my second evaluation. We had a very productive talk about the medications used for rapid sequence intubation such as etomidate and succinylcholine, and the differences between them and other medications like rocuronium or fentanyl. The journal article I presented dealt with the diagnostic accuracy of the eFAST exam, and how it can be useful to rule in diagnoses but is not sensitive enough to rule out diagnoses, and false negatives may be present. Shterni provided very helpful advice for pursuing a career with EM (and dealing with intoxicated patients), and her constructive feedback for my HPIs was very helpful for future write-ups.
